Juntos Wellness (also referred to as “us” or “we”) is one of many projects that engage in charitable and/or educational activities, hereinafter referred to collectively as the “Service(s).”
We take your privacy and information very seriously. To that end, we have this Privacy Policy to advise you of our rules on handling the information we receive connected to the Services. By accessing and using the Services of Community Initiatives or this Site, you also agree to our Privacy Policy in addition to the posted terms and policies of Community Initiatives, and any other Project’s Site(s) you visit as explained below. If you do not agree to abide by this Privacy Policy, please immediately exit the Site and stop using the Services. Please also review our other terms, such as our Terms of Service, as these also dictate how you may use our Services. The Services include operating the website found at https://www.juntoswellness.net/about (also referred to below as the “Site” or “Website”).
This Privacy Policy is effective as of the effective date noted above and will remain in effect until we change it. We reserve the right to change this Privacy Policy at any time and will post changes to this policy on our Site. You should check this policy periodically as its terms may change from time to time. If you continue to use the Services, that means you acknowledge and agree to the modified policy.
Juntos Wellness is a project under a fiscal sponsor, Community Initiatives, whose website can be found here. Being a fiscal sponsor means Community Initiatives is a California nonprofit public benefit corporation that is recognized as tax-exempt under federal and state law. Each Project engages in distinct charitable and/or educational activities as programs of and overseen by Community Initiatives. As such, while Community Initiatives has a broad privacy policy, which can be found here, the details of our privacy policy may vary, and are focused on how we collect and use information identifying or identifiable to individuals (“Personal Information”) in pursuit of our charitable and/or educational activities.
As a public charity, the majority of privacy laws are not applicable to us. We also do not collect an amount of data from Oregon, Colorado, or New Jersey, which would render us subject to the omnibus privacy laws of those states. If this were ever to change, this privacy policy would be updated to reflect those new obligations.

1. Information We Collect, How We Use It, Where We Share or Sell
This section explains how Juntos Wellness uses, discloses, exchanges, or transfers the Personal Information you provide on this website or that we collect through other means. Juntos Wellness collects Personal Information both through 1) your interactions with us, including our website, 2) shared with us from our fiscal sponsor Community Initiatives, and 3) from our Service Providers (also referred to as “processors” under some privacy laws).

What Personal Information is Collected by Juntos Wellness and How it Is Used
To clearly explain how and why your Personal Information is collected and used, we have created the list of collected Personal Information found below. We do not sell or disclose any of your Personal Information to any third party.

Identifiers and Personal Information Categories
- What We Collect: Juntos Wellness collects the following identifiers from users of our Website and through interactions with the communities we service: First name, last name, physical address, email address, phone number, date of birth, signatures, employment history, gender, and household information.
- What We Do With It: Information collected from you on our Website is collected for purposes of operating the website. It is also used to allow us to respond to inquiries, to communicate with you (for example when you contact us for information), for analytics and internal marketing purposes, and for fraud prevention. This information is used to provide the expected service of the Website and in support of our charitable mission. Information collected offline is used in support of our charitable mission.
Sensitive Information
- What We Collect: Juntos Wellness collects the following sensitive information as part of our charitable mission: physical and mental health assessments, sexual history.
- What We Do With It: Sensitive information is collected for purposes of furthering our charitable mission, including but not limited to mental health services and youth development opportunities for Lennox School District students.
Children’s Information
- What We Collect: Juntos Wellness collects the following information from children under the age of 13 as part of our charitable mission: name, physical address, phone number, date of birth, signatures, employment history, gender, household information, physical and mental health assessments, sexual history.
- What We Do With It: Sensitive information is collected strictly for purposes of furthering our charitable mission, including but not limited to mental health services and youth development opportunities for Lennox School District students.
Use of Data Generally, Service Providers of Juntos Wellness
In addition to the foregoing, we use Personal Information for purposes of conducting our charitable activities. If you make an inquiry, we may use your Personal Information to contact you with updates or other information that we think may be of interest to you. You may opt out of these communications at any time, excluding emails directly relating to any ongoing business we have together.
We will never sell or otherwise provide your information to any unaffiliated third parties without your prior consent. We may disclose information when legally compelled to do so, when we, in good faith, believe that the law requires it, or for the protection of our legal rights.
Juntos Wellness may aggregate information about you and other individuals together so that the information does not identify you personally. Juntos Wellness may use information in these forms for any legitimate business purpose, including for research and analysis, administration of services, advertising and promotional purposes.
Juntos Wellness uses Service Providers to assist with email communications, event registrations, on-site payments, online donations, and marketing purposes. Juntos Wellness may share your Personal Information with these Services Providers to assist with the purposes outlined in this Privacy Policy.
We will not collect additional categories of personal information or use the personal information we collect for materially different, unrelated, or incompatible purposes without providing you notice.
2. Sources of Information
We obtain the personal information listed above from the following categories of sources:
- Directly from users of our Website and other consumers. For example from forms you complete, donations you make, or in relation to services you receive from us.
- Indirectly from users of our Website through website usage details we collect. For example, from observing your actions on our Website through analytics tools.
- In person, from students and parents.
3. Sensitive Information
Juntos Wellness does not use or disclose any sensitive information on you except to perform services reasonably expected by you, for fraud detection purposes, and to detect security incidents–for example, the use of payment information when you are making a donation to us.
However, the services we provide, including mental health services, require the collection of some types of information considered sensitive under privacy laws. As such, we do collect sensitive information on individuals for the purposes of providing the services to these communities. Sensitive information is defined by GDPR as Personal Information that reveals any of the following: (1) personal data revealing racial or ethnic origin, (2) political opinions, religious or philosophical beliefs; (3) trade-union membership; (4) genetic data, biometric data processed solely to identify a human being; (5) health-related data; or (6) data concerning a person’s sex life or sexual orientation.
4. If You Live Outside of the United States
Juntos Wellness’s website is hosted in the United States and is governed by United States law. If you are using the website from outside the United States, please be aware that your information may be transferred to, stored and processed in the United States. The data protection and other laws of the United States might not be as comprehensive or protective of certain rights as those in your country. Under United States law, your information may be subject to access requests from governments, courts, or law enforcement in the United States. By using the website, you consent to your information being transferred to our facilities and to the facilities of those service providers with whom we share it as described in this Privacy Policy and processed in those facilities.
5. Data Retention and Security
We will retain your Personal Data only for as long as is necessary for the purposes set out in this Privacy Policy. We will retain and use your Personal Data to the extent necessary to comply with our legal obligations (for example, if we are required to retain your data to comply with applicable laws), resolve disputes, and enforce our legal agreements and policies.
For users that register on our website (if any), we also store the personal information they provide in their user profile. All users can see, edit, or delete their personal information at any time (except they cannot change their username). Website administrators can also see and edit that information.
We will also retain Usage Data for internal analysis purposes. Usage Data is generally retained for a shorter period of time, except when this data is used to strengthen the security or to improve the functionality of our Service, or if we are legally obligated to retain this data for longer time periods.

9. Children's Privacy and Information of Children
As a public charity, the requirements of the Children’s Online Privacy Protection Act generally do not apply to Juntos Wellness. However, Juntos Wellness takes the privacy of children extremely seriously and, where possible, endeavors to ensure that additional protections for children’s data are extended in line with common standards.
Juntos Wellness specifically provides health and wellness services to students below the age of 13, preschool to grade 8, including mental health services and youth development opportunities for Lennox School District students. As part of the services inherent to Juntos Wellness’ charitable mission, Juntos Wellness collects some information from children who may be part of particularly sensitive or vulnerable groups in furtherance of our charitable mission. This information is processed under a lawful basis and treated with a high degree of confidentiality and care, with a focus on protecting the children in question. Juntos ensures parental consent before collecting the data of any minors.

10. HIPAA/FERPA Statement
As noted above, we provide services furthering our charitable mission, including but not limited to mental health services and youth development opportunities for Lennox School District students. In performing these services we have access, either directly through services provided to a specific school, to some information potentially covered under the Health Insurance Portability and Accountability Act (“HIPAA”) and/or the Family Educational Rights and Privacy Act (“FERPA”), as a business associate. All such information is collected and used in compliance with applicable law.
